After you have completed this Mail Ballot Application, mail it as soon as possible to:

Once Cook County verifies your registration and process your application, they will send a paper ballot to the mailing address you designate in your application. 

Ballots will be mailed from Cook County - TBA.

If you are unable to download an application and need this application mailed to you, please call the Clerk's Office at (708) 403-6150 and staff will be happy to drop an application in the mail to you!

Want to Save a Stamp! - Apply Online for Mail Ballot Application

Applying online takes less than a minute.  This process eliminates two or three legs of the "snail mail" process, so it is the most efficient method for voters and the Cook County Clerk's election division.

The mail ballot application can be completed on either computers or smartphones/tablets. 

  • From your desktop or laptop computer, visit cookcountyclerk.com/votebymail. Click “Apply,” read the instructions and then click “Continue.”
  • From your smartphone or tablet, visit m.cookcountyclerk.com. Select the “Search By Birthday” tab and begin entering your information.

To complete an online mail ballot application, you will need:

  • an email address
  • your driver’s license or state identification number
  • the last four digits of your Social Security Number
  • and the address where you would like your ballot mailed

Any registered suburban Cook County voter may request a mail ballot using Cook Counties online Mail Ballot Application.

**The deadline to apply for a mail-in ballot is five days before an election which will be TBA**
